More than a dozen states require insurance companies to offer varying forms of benefits for infertility treatment. Generally these requirements or mandates are of two kinds: to "cover" treatments and to "offer to cover" infertility treatments. A mandate "to cover" means that a law requires insurance companies to provide a certain kind of benefits. They provide some level of infertility treatment benefit in every policy and include the cost in the policy premium. Infertility treatment benefits vary from one state to another. Basically coverages include comprehensive infertility diagnosis and treatment which takes into consideration assisted reproductive technology procedures covering the cost of in vitro fertilization, ovulation induction, intrauterine insemination, embryo transfer and some others. Infertility Insurance is accessible to individuals in every state in case they meet all the requirements mandated by the law of the state they live in regarding their age, the conditions that don't let them conceive or sustain a successful pregnancy, history of infertility treatment. There are states (California, Connecticut, Texas) where the law requires to offer coverage for infertility treatment. Insurers offer employers a policy for this provision but employers may choose to purchase it or not.
